For Trafford professionals making a referral:
Alongside completing Home‑Start’s online referral, you must also complete a Strengths and Needs Form (which replaces the Early Help Assessment). This form gives a holistic overview of the family’s situation, their strengths, their needs, and the specific support requested from Home‑Start.
- If you use Liquid Logic: complete the Strengths and Needs Form on EHM. For any issues, contact: Liquidlogicsupport@trafford.gov.uk.
- If you do not have access to Liquid Logic: download the form Your Family’s Strengths and Needs and email it to admin@hsts.org.uk.
You may submit the online referral without the form, but please note you will be contacted and asked to complete one.
